1. ASSUMPTION OF RISK
I, the undersigned, acknowledge that participation in art classes, workshops, camps, open studio time, and related activities (collectively, “Activities”) offered by Misaki Chaidez d/b/a Master Art Academy, LLC (“Master Art Academy”) involves inherent risks.
These risks may include, but are not limited to:

Use of art materials, tools, and equipment (including scissors, paints, brushes, clay tools, easels, furniture, and
cleaning supplies)

Slips, trips, or falls

Accidental contact with other participants

Allergic reactions or sensitivities

Minor injuries or property damage
I voluntarily assume all known and unknown risks, even if arising from the ordinary negligence of Master Art Academy, to the fullest extent permitted under California law.
2. RELEASE OF LIABILITY (CALIFORNIA COMPLIANT)
To the fullest extent permitted by California law, I hereby release, waive, discharge, and covenant not to sue Misaki Chaidez, Master Art Academy, LLC, and their owners, officers, employees, instructors, assistants, volunteers, agents, and representatives (collectively, “Released Parties”) from any and all claims, demands, actions, or causes of action arising out of or related to participation in the Activities.
This release applies to ordinary negligence only and does not apply to gross negligence, reckless conduct, or intentional misconduct, which cannot be waived under California law.
3. PARENT / GUARDIAN AGREEMENT FOR MINORS
If the participant is under the age of 18, I certify that I am the parent or legal guardian of the minor participant. I consent to the minor’s participation and agree to all terms of this Release on behalf of myself and the minor.
I further agree to indemnify and hold harmless the Released Parties from any claims brought by or on behalf of the minor arising out of participation in the Activities, except where prohibited by law.
4. MEDICAL CONSENT
certify that the participant is physically able to participate in art activities and has no medical condition that would prevent safe participation, except as disclosed in writing to Master Art Academy.
I understand that participation in art classes and related activities involves inherent risks, including but not limited to minor injuries such as cuts, slips, or allergic reactions. I voluntarily assume all risks associated with participation.
In the event of injury or illness, I authorize Master Art Academy, its owners, instructors, employees, and agents to obtain emergency medical treatment for the participant as reasonably necessary. I understand and agree that I am financially responsible for any medical care provided.
I hereby release, waive, and hold harmless Master Art Academy, its owners, instructors, employees, and agents from any and all claims, demands, or causes of action arising out of or related to the participant’s participation in art activities, except to the extent caused by gross negligence or willful misconduct.
5. PHOTO & VIDEO RELEASE
I grant permission for Master Art Academy to photograph or record the participant during activities and to use such images or recordings for educational, promotional, or marketing purposes, including on websites and social media, without compensation.
6. DIGITAL SIGNATURE & ELECTRONIC RECORDS CONSENT
I understand and agree that this Release may be signed electronically. I consent to the use of electronic signatures, electronic records, and digital acknowledgment.
I agree that my electronic signature is the legal equivalent of my handwritten signature and is enforceable under the California Uniform Electronic Transactions Act (UETA) and the Federal ESIGN Act.
7. GOVERNING LAW & SEVERABILITY
This agreement shall be governed by and interpreted under the laws of the State of California.
If any provision is found unenforceable, the remaining provisions shall remain in full force and effect.
